RH Haven Community Wellness and Support Program

RH Haven provides trauma-informed psychosocial support through one comprehensive program- RH Haven Community Wellness and Support Program. All services are included, and each client receives a customized plan based on their needs, goals, strengths, and daily living challenges. RH Haven Community Wellness and Support is delivered one-on-one using the RH Haven Community Wellness System to ensure consistency and measurable progress.


Services included in the program include:

Psychosocial Rehabilitation and Skill Building

This core service focuses on emotional, social, and daily living skills that help individuals remain stable and active in the community. Each client receives customized skill-building based on their functional needs.

Areas of support may include:

• Emotional regulation and coping skills
• Responding to voices and symptoms
• Motivation and follow-through
• Social skills and communication
• Problem-solving in real-life situations
• Anger management strategies
• Relapse prevention and substance use coping support

Sessions take place in the home or community.

Community Integration and Outings

Clients practice real-life skills in real-world environments to build confidence and reduce isolation.

Examples include:

• Grocery shopping and price comparison
• Planning transportation routes and attending appointments
• Visiting parks, libraries, and job centers
• Practicing social skills in public settings
• Managing anxiety outside the home
• Navigating community resources independently

Emotional Wellness Skill Building

These sessions focus on emotional wellness and thinking patterns that affect daily functioning.

Focus areas may include:

• Identifying emotions and thoughts
• Coping tools for stress, panic, or voices
• Grounding, self-soothing, and self-talk strategies
• Anger triggers and response control
• Substance use urges and coping plans
• Building routines that support mental health
• Creating simple action plans for daily stability

Life Skills and Independent Living Support

Clients receive practical support to build independence and confidence in daily living.

• Meal planning and simple shopping lists
• Basic nutrition and food choices
• Budgeting, tracking spending, needs versus wants
• Hygiene routines and personal care
• Time management and planning
• Medication reminders and appointment calendars
• Home organization and basic housekeeping

Grief and Loss Support

Clients experiencing grief or major life loss may receive additional support focused on emotional processing and routine rebuilding.

• Creating new meaning and purpose

Relocation Support

The Relocation Support Module is designed for individuals who have recently moved or are adjusting to a new environment and are struggling with routines, emotional stress, or grief related to the transition. Relocation often brings hidden losses and overwhelming changes that affect daily functioning and stability. This module focuses on rebuilding structure and confidence in a new setting.

Ideal for:

• newcomers to new areas
• immigrants and individuals relocating from another country or state
• individuals who relocated due to illness or caregiving
• those who moved after separation or divorce
• families leaving shelters or transitional housing

Focus areas include:

• understanding relocation grief and transition stress
• clarifying needs and direction in the new environment
• establishing routines and daily structure
• navigating community resources and services
• building new community connections and support systems
• emotional regulation during change
• gentle action planning for successful adjustment

This module includes grief and stress support as part of the relocation process, recognizing that relocation often involves the loss of familiar people, routines, roles, and identity.

Fresh Start Support

The Fresh Start Support Module is designed for individuals rebuilding after major life changes who need structure, emotional stability, and a clear path forward. This module helps clients restart with purpose and confidence.

• individuals leaving a difficult season
• those recovering from burnout or major setbacks
• individuals seeking a new direction in life
• people redefining themselves after crisis or transition

Focus areas include:

• clarifying current season and personal goals
• creating routines and stability at home and in the community
• emotional regulation during change
• rebuilding identity and confidence
• establishing supportive relationships and community connections
• gentle action planning

Additional support may include:

• career exploration and job readiness
• building financial stability skills
• connecting to employment resources
• budgeting and income planning
• identifying simple income pathways

Stress Management & Support

Clients experiencing ongoing stress that affects their daily functioning may receive focused support to build resilience and stability. Many individuals struggle with overwhelming thoughts, pressure, and emotional overload that interrupt routines and increase crisis risk.

Examples include:

• daily overwhelm
• caregiving stress
• financial or housing pressure
• medical stress
• work or family stress

Focus areas may include:

• identifying stress triggers
• learning calming and grounding strategies
• building routines that reduce overwhelm
• creating simple coping plans for stressful moments
• improving time management and prioritizing tasks
• strengthening support systems

Substance Use Support and Relapse Prevention

For clients with a history of substance use, the program provides psychosocial support focused on:

• identifying triggers
• building coping strategies
• strengthening support systems
• reducing isolation
• preventing relapse through daily structure

This is support-based, not treatment-based, and fits within PSR guidelines.

Anger Management Support

Clients who struggle with anger responses that affect relationships, housing, or community placement may receive support to understand and manage emotional reactions. The goal is to help individuals feel more in control and reduce the negative impact anger has on daily life.

Examples include:
